We did an Italy trip last summer and worked with Pina Esposito (
https://www.privateguide.com/en/pina/) for transportation and tours.
We flew into Naples and had Pina arrange a driver to pick us up, take us to a pizza place, then to the Archaeological Museum and wait for us. From there, he took us to Vesuvius, then to our hotel in Pompei. I think it was about 3 days before our trip that we decided to add Vesuvius and she was able to make it happen without issue.
The next day, she arranged a driver to pick us up at our hotel and take us to Pompeii, lunch, and Herculaneum. Pina was our guide for the full day - including at lunch where she made recommendations and showed us how to eat pizza like a local. Pina really shined in Pompeii and Herculaneum. She's very passionate about the ruins, has studied them in her degree programs, and knew how to avoid the crowds (if she saw a crowd headed to where we were going next, she'd encourage us to 'seize the day, andiamo' and walk a bit faster to get in front of them.

)
The next day, she arranged a driver to take us through the Amalfi area. Unfortunately, she had a family emergency and couldn't join us, but she worked with one of her local guide friends and he joined us for the day instead.
Finally, she arranged a driver to take us from hour hotel in Pompei to our hotel in Rome.
I found her in the Rick Steves Italy book and she came highly recommended from the Rick Steves world.
Her communication is extremely detailed - some of her mails are a good 5 minute read. Her English was perfect. The drivers struggled with English, but when Pina wasn't with us, we could still communicate with them through very basic words in English or Italian, using hand signals and charades, Google Translate, or, in two situations, the driver called Pina to get her help translating.
Her tours are private, so that comes at a price. We had a group of 4, so the price difference wasn't too bad. Having the private tour allowed us to do whatever we wanted when we wanted to do it.
